RFNSW provides support to people with a mental illness. RFNSW will ask you (or someone else on your behalf) to fill out a Referral form to return to us. Click here for a copy of our Referral form.

Please understand that entry to our service is not always immediate and there can sometimes be a wait.

We need to know a few things about you:

• Your mental illness, your treatment and the medication you are taking.
• What sort of things you need help with (for example: shopping, cleaning, using public transport, getting to appointments).
• Where you currently live and how we can contact you.

Start by reading through the RFNSW website and see if it answers your questions. If you are still unsure, you can call us for an information pack or speak with someone by telephone. Email is also a good way to contact us, and you’ll receive a reply within 24 hours.

If you want to take the next step, you can fill out a Referral Form or have someone else do it for you and send it to us. Once we have received your Referral, we will contact you and let you know what to do next.
